In early 2021, CommonSpirit Health Population Health developed an internal campaign to encourage physicians to reach out to patients with high blood pressure (hypertension). After a year of the pandemic, the health system was seeing higher rates of uncontrolled blood pressure, and to help improve the health of our populations, it was important to get those numbers back in a healthy range. We developed collateral for our physicians, educating them about the issue and provided patient-facing materials for them to use in their comms efforts.
